Job summary

Altus Hospice Care is seeking a dedicated Hospice Registered Nurse Case Manager (RNCM) to join our compassionate team in Southeast Houston. You will provide hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their journey.

The role requires an active RN license, at least 1 year of relevant nursing experience, EMR proficiency, CPR as required, and a valid driver's license. On-call duties and a regular Monday–Friday schedule are included.

Qualifications

• Must hold current unencumbered RN license.
• At least 1 year RN experience in medical-surgical/acute care; hospice exp is a plus.
• Proficient with computer systems and EMR.
• CPR certification may be required by state.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess client eligibility.
• Deliver compassionate direct care to patients and families.
• Apply nursing skills to ease pain and improve quality of life.
• Educate and empower patients and families for personalized care plans.
• Participate in on-call duties as part of the team.
