Job summary

Altus Hospice Care is seeking a Full Time Hospice Registered Nurse Case Manager to join our compassionate team serving Corpus Christi and surrounding communities. You will deliver hands-on hospice nursing care, coordinate services, and guide families through every step of the journey.

We value compassion and clinical excellence; RN license, 1 year in acute care, EMR proficiency, CPR as required, and a valid driver’s license are essential.

Qualifications

• Active Registered Nurse license in good standing.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with EMR systems and basic computer skills.
• CPR certification required if state mandates.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services.
• Guide patients and families through the hospice journey with compassion.
• Collaborate with Clinical Director to assess eligibility for services.
• Provide education and advocacy to patients and families.
• Participate in on-call duties as needed.

Altus Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Altus Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule

Monday - Friday, 8 am - 4:30 pm. Occassional On-Call Evenings, Weekends and Holidays Required.

Territory

Corpus Christi, Rockport, Calallen, and Robstown.

Responsibilities

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
• Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care, at the right time.
• Nurturing Direct Care: As an RN Case Manager, you're not just coordinating care – you're a source of comfort and support for patients, ensuring their comfort and quality of life remain paramount.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$76,000 - $87,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
