Job summary

Altus Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our dedicated nursing team throughout the Southeast Houston area. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.

This role reports to the Clinical Director with a Monday–Friday 8 am–4:30 pm schedule and on-call included. We offer a comprehensive benefits package, PTO, holidays, tuition assistance, wellness

Qualifications

• Current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
• Proficient computer skills and EMR experience.
• CPR certification (as required by state).
• Valid driver's license and reliable transportation.

Responsibilities

• Provide hands-on hospice nursing care in patients' homes or facilities.
• Coordinate services and communicate with interdisciplinary care team.
• Guide patients and families through the hospice journey with compassion.
• Deliver care that focuses on comfort, quality of life, and symptom management.
• Educate and empower patients and families to participate in care planning.
• Share call duties as part of the team.

Altus Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
• Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care, at the right time.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$76,500 - $87,500 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
