Job summary

Frontier Hospice in Edmond, OK is seeking a full-time Hospice Registered Nurse Case Manager to join our dedicated nursing team delivering compassionate end-of-life care throughout the community. As an RN Case Manager you’ll provide hands-on nursing care, coordinate services, educate patients and families, and collaborate with our Clinical Director to ensure timely, appropriate care.

Qualifications

Current unencumbered RN license.
Minimum 1 year RN experience in med-surg/acute care; hospice a plus.
Proficient with EMR and computer tasks.

Responsibilities

Collaborative Care Champion: work with Clinical Director to assess eligibility for hospice services.
Nurturing Direct Care: provide hands-on nursing care focused on comfort and quality of life.

Educator and Advocate: educate patients and families to create personalized care plans.
Call of Compassion: participate in on-call duties as part of the team.
