Job summary

Frontier Hospice is hiring a Full Time Hospice RN Case Manager (RNCM) to provide hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey. Schedule is Monday–Friday, 8 am–4:30 pm across Pauls Valley, Purcell, Noble and Midwest City.

Qualifications

Hold RN license in good standing and able to practice in the state.
Minimum 1 year of RN experience in medical-surgical/acute care; hospice a plus.
Proficient with EMR systems and basic computer tasks; CPR may be required.

Responsibilities

Deliver hands-on hospice nursing care and coordinate services for patients.
Collaborate with the Clinical Director to assess eligibility for hospice services.
Provide education and support to patients and families to empower care decisions.
Apply nursing skills to ease pain and improve quality of life for patients.
Participate in call duties as part of the interdisciplinary team.
