Job summary
Sanctuary Hospice is seeking a full-time Hospice Registered Nurse Case Manager to support patients and families in the Cincinnati area. You will deliver hands-on nursing care, coordinate services, and guide families through the hospice journey with compassion and clinical excellence.
The role follows a Monday–Friday schedule, 8:00 am–4:30 pm, with a focus on education, advocacy, and teamwork. Sanctuary Hospice offers robust benefits and professional growth opportunities.
Qualifications
- Hold current unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
- Proficient in typing and computer programs; EMR background.
- Must hold CPR certification if state requires.
- Reliable transportation with valid license and insurance.
Responsibilities
- Respond to referrals with compassionate, informed care.
- Collaborate with Clinical Director to assess client eligibility for hospice services.
- Provide hands-on nursing care and coordinate services to maintain comfort and quality of life.
- Deliver nursing care rooted in established clinical principles.
- Educate patients and families to develop personalized care plans.
- Participate in on-call duties as part of the team.
