Sanctuary Hospice is seeking a Full-Time Hospice Registered Nurse Case Manager to join our compassionate team serving Cincinnati and surrounding areas. You will coordinate care, guide patients and families, and deliver hands-on hospice nursing in a community setting.

The role emphasizes clinical excellence, patient comfort, and education; you’ll work with an interdisciplinary team and participate in on-call duties as needed. Comprehensive benefits accompany the position.

Qualifikationen

• Current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience is a plus.
• CPR certification if required by state.
• Valid driver license, vehicle insurance, and reliable transportation.
• Proficient in typing and EMR systems.

• Respond to referrals with care and expertise, addressing inquiries with compassion.
• Collaborate with Clinical Director to assess eligibility and determine appropriate care.
• Provide direct nursing care and ensure patient comfort and quality of life.
• Deliver nursing skills rooted in proven principles to ease pain and suffering.
• Educate patients and families to participate in personalized care plans.
• Participate in call duties as part of team responsibilities.
