Job summary
Legacy Hospice seeks a compassionate Registered Nurse Case Manager (RNCM) to provide hands-on hospice nursing care and coordinate services for patients in the Charlottesville area. This full-time role follows a Monday–Friday, 8:00 am–4:30 pm schedule as part of an interdisciplinary care team, guiding patients and families through every step of their hospice journey.

Qualifications
- Current, unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
- Proficient computer skills; background in EMR.
- CPR certification as required by state.
- Valid driver’s license and reliable transportation.
Responsibilities
- Respond to referrals with care and expertise, addressing inquiries compassionately.
- Collaborate with Clinical Director to assess client eligibility for hospice services.
- Deliver direct hospice nursing care and coordinate services to maintain comfort and quality of life.
- Provide skilled nursing with proven clinical methods to ease pain and promote comfort.
- Educate and advocate for patients and families to participate in personalized care plans.
- Participate in on-call duties as part of the team.
