Job summary

Legacy Hospice in Charlottesville is seeking a full-time Hospice RN Case Manager to join our compassionate nursing team. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through their hospice journey.

The schedule is Monday–Friday, 8 am–4:30 pm. We value clinical excellence, compassionate care, and teamwork, with a thorough onboarding and strong benefits package.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in a medical-surgical/acute care setting; hospice exp a plus.
• Proficient with EMR and computer applications.
• CPR certification if required by state.
• Current driver's license and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct nursing care, focusing on comfort and quality of life.
• Deliver nursing care rooted in proven principles to ease pain.
• Educate patients and families to support personalized care plans.
• Share call duties to ensure availability when it matters most.
