Job summary

Buckeye Hospice is seeking an RN Case Manager to oversee the assessment and care of hospice patients in nursing facilities and home settings across Youngstown and surrounding locations.

The role requires an RN license, a valid driver’s license, and a minimum of 1–4 years in hospice or home health settings. On-call rotation is possible. This full-time position offers the opportunity to coordinate comprehensive patient care in a supportive team environment.

Qualifications

• RN license required.
• Valid Driver's License and reliable insured transportation.
• On-call rotation may be required.

Responsibilities

• Provides initial and comprehensive nursing assessment of patient needs.
• Implements, reevaluates and revises the care plan to meet patient nursing needs.
• Communicates patient progress with patient, caregiver and other disciplines; directs and instructs care staff to deliver quality care.
• Assesses and cares for hospice patients in nursing facilities and home settings.
