Job summary

1st Care Palliative and Hospice Services seeks an experienced RN Case Manager for Hospice in Pottsville, PA. You will coordinate and deliver hospice nursing care across counties, including assessments, care plans, medications, and symptom management.

You will collaborate with an interdisciplinary team, communicate with physicians and families, support discharge planning, and educate patients, families, and caregivers about hospice services and options.

Qualifications

• RN with hospice/palliative care experience preferred.
• Experience in Case Management and Discharge Planning to coordinate services and transitions.
• Knowledge of Utilization Management to ensure efficient resource use and patient-centered care.
• Active RN license in Pennsylvania.
• Willingness to travel locally as needed.
