Job summary

The ideal candidate has an RN license in PA, at least 1 year in hospice and leadership experience, and a willingness to travel daily within the nearby counties. This is a full-time position with standard daytime hours and on-call requirements.

Qualifications

• High School Diploma or GED required.
• 1+ year of Hospice experience.
• 1+ year of Management or leadership experience.
• 1+ year in Home Health, Epic Home Health, or Home Health.
• Active RN license in PA and BLS/ACLS certifications.

Responsibilities

• Manage and oversee patient care in hospice settings.
• Coordinate care plans with interdisciplinary teams.
• Provide leadership and mentorship to nursing staff.
• Ensure compliance with hospice standards and state regulations.
• Deliver patient-centered, compassionate care to patients and families.
