Job summary

The Pennant Group, Inc. is seeking a registered nurse to plan, organize and direct hospice care in home settings. The RN brings community health experience to assess and meet patient and family needs within homes and communities.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years recent acute care experience.
• 1–2 years home care experience.
• Current licensure in State and CPR certification.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family.
• Provides professional nursing care using the nursing process.
• Initiates and revises the plan of care as patient status changes.
• Acts as Case Manager and coordinates patient care.
• Maintains accurate clinical notes and communicates with physicians.
• Teaches self-care to patient and family and coordinates with interdisciplinary team.
• Participates in on-call duties and ensures equipment/services are available.
