Role Description The RN Case Manager for Hospice is a full-time, on-site role based in Pottsville, PA. This role will be responsible for caring for patients in Northumberland County, and Schuylkill County Pennsylvania. This role is responsible for coordinating and delivering hospice nursing care, including comprehensive assessments, developing individualized care plans, administering medications, and providing symptom management. The RN Case Manager will collaborate with an interdisciplinary team to ensure continuity of care, communicate with physicians and families about patient status, and support discharge planning and transitions across care settings. Daily tasks include ongoing case management, utilization of resources to meet patient needs, documentation of clinical interventions, and participation in team meetings. The RN Case Manager will also provide education to patients, families, and caregivers about hospice services, care options, and comfort measures.

Qualifications

• Strong clinical foundation in Nursing and Medicine, with the ability to deliver high-quality hospice and palliative care.
• Experience in Case Management and Discharge Planning to coordinate services and support safe, appropriate transitions of care.
• Knowledge of Utilization Management to ensure effective use of resources while maintaining patient-centered care.
• Active Registered Nurse (RN) license in the state of Pennsylvania.
• Previous hospice, palliative care, or home health experience preferred.
• Excellent communication, interpersonal, and patient advocacy skills.
• Ability to work collaboratively in an interdisciplinary team and independently in the field.
• Reliable transportation and willingness to travel locally as needed.
• Basic computer and electronic health record (EHR) documentation skills.
