Suncoast Hospice of Hillsborough, a member of Empath Health, is seeking an RN Case Manager to join the Empath team. Monday - Friday, daytime hours, coverage area North Hillsborough with direct patient care in facilities.

You will build meaningful relationships with patients and families, guiding their hospice journey with compassion, expertise, and dignity. As RN Case Manager, you’ll coordinate with the IDT, develop and update the Plan of Care, supervise LPNs and Home Health Aides, and provide

Qualifications

• Associate’s degree (A.A.) or equivalent from two-year college or technical school.
• Valid Florida RN license.
• Valid Florida State Driver’s License.
• Bilingual Spanish/English preferred.
• For field-based positions, reliable transportation and proof of auto insurance.

Responsabilités

• Assess patient history and perform age-specific physical, psychosocial and spiritual assessments and initiate referrals and collaboration with IDT and facilities.
• Develop and monitor Plan of Care with patient, family, IDT, attending and facility, updating as needed.
• Direct care coordination and ongoing evaluation of service provision.
• Provide patient/family teaching and administer medications as prescribed.
• Assist with closure contact and support after a patient’s death in collaboration with IDT.
