Bristol Hospice is seeking a Registered Nurse Case Manager to provide compassionate, holistic care in Napa, CA. The role emphasizes advocacy, family support, and a patient-centered journey through hospice services.

The RN will develop individualized care plans, monitor vital signs, administer medications, and educate families while collaborating with interdisciplinary teams in a mission-driven environment.

Qualifications

• Requires RN license in good standing in the state of practice.
• CPR certification (current) is required.
• Valid driver\'s license and reliable transportation.
• Strong observational and written/verbal communication skills.

Responsibilities

• Make a true difference - one patient, one family, one compassionate moment at a time.
• Develop and implement individualized care plans based on medical history.
• Deliver attentive, one-on-one support to patients and families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely responses.
• Keep up-to-date clinical notes reflecting patient progress and quality of care.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Contribute to other supportive duties as part of a compassionate, mission-driven team.
• Additional duties as assigned.
