Bristol Hospice in Mississippi is seeking a Registered Nurse Case Manager who provides compassionate, whole-person care and advocates for patients and their families throughout the hospice journey. You will develop individualized care plans, monitor patient conditions, educate families, and collaborate with interdisciplinary teams to ensure comfort, dignity, and timely symptom management.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver attentive, one-on-one support to patients and families, ensuring comfort, dignity, and understanding.
• Monitor and document vital signs and patient progress.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Keep detailed clinical notes reflecting patient progress and quality of care provided.
