Qualifications

• Active Registered Nurse license in good standing for the state of employment.
• Current CPR certification.
• Excellent written and verbal communication skills.
• Reliable transportation with valid driver’s license.

Responsibilities

• Provide compassionate, patient- and family-centered hospice care.
• Develop and monitor individualized care plans.
• Administer physician-ordered treatments and medications.
• Document patient status and changes in care; update care plans.
• Collaborate with interdisciplinary team to coordinate care.
