Job summary

Hinds LifeCare in Fresno, CA is seeking a Full Time RN Case Manager to provide skilled palliative and end-of-life care in the community. You will coordinate an interdisciplinary plan and manage hospice services for patients and families, promoting the hospice philosophy focused on quality of life.

The role requires an RN license in California, CPR certification, and availability for a 8:00am-4:30pm schedule with on-call rotation.

Qualifications

• Current RN Licensure and CPR Certification.
• One year experience in acute care facility or community health agency; hospice experience a plus.
• Valid driver’s license, auto insurance, reliable transportation.
• Ability to meet the physical demands of the job (Push, Pull, Lift/Carry 35 lbs).
• Available to work 8:00am-4:30pm M-F with on call rotation.
