The role emphasizes compassionate, individualized care, vital sign monitoring, medication administration, and thorough documentation in a supportive home- or facility-based setting.

Qualifications

• RN license in good standing in the state you work in.
• CPR certification current.
• Compassionate, patient-centered care for hospice.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver compassionate, one-on-one care to patients and families.
• Develop and implement individualized care plans.
• Monitor vital signs and document findings accurately.
• Administer physician-ordered medications and treatments.
• Educate patients and families on care techniques and symptoms.
• Collaborate with interdisciplinary team to ensure comprehensive care.
• Track changes in patient condition and respond promptly.
• Maintain detailed clinical notes reflecting patient progress.
• Support other duties as part of a mission-driven team.
