Job summary

Bristol Hospice Modesto, CA is seeking a Registered Nurse Case Manager to provide compassionate, patient- and family-centered care across the hospice journey. You will be integral in advocating for patients and guiding families through challenging moments.

You will develop individualized care plans, monitor vitals, administer treatments, educate families, and collaborate with interdisciplinary teams to deliver dignified, comfort-focused care.

Qualifications

• Valid RN license in good standing in the state you will work in.
• Valid driver's license and reliable transportation.
• CPR certification (current).
• Strong observational and written and verbal communication skills.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support to patients and families with comfort and dignity.
• Conduct assessments of physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition and respond promptly.
• Keep up-to-date clinical notes reflecting patient progress and care quality.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Support additional duties as part of a compassionate, mission-driven team.
