Bristol Hospice Visalia is seeking an RN Case Manager to deliver compassionate, full-time care in Visalia, CA. You will develop care plans, monitor patients, and support families with empathy, documenting progress and coordinating with interdisciplinary teams.

Join a mission-driven team dedicated to high-quality end-of-life care and patient comfort, with a Monday–Friday schedule and a $49.00–$52.00 hourly wage. This role emphasizes presence, advocacy, and holistic care.

Qualifications

• Valid RN license in good standing in the state of employment.
• CPR certification (current).
• Valid driver’s license with reliable transportation.
• Strong observational and written and verbal communication skills.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver attentive, one-on-one support to patients and families with comfort and dignity.
• Conduct assessments of physical, emotional, and spiritual needs along the care journey.
• Monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.
• Track and communicate changes in patient condition for timely responses.
• Keep detailed clinical notes reflecting patient progress and quality of care.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Support additional duties as part of a compassionate, mission-driven team.
