Bristol Hospice is hiring an RN Case Manager in Fresno, CA. Join a mission-driven team providing compassionate, whole-person hospice care.

This full-time, day-shift role offers competitive hourly pay and benefits. You will partner with patients and families to develop care plans, assess needs, document progress, and coordinate with interdisciplinary teams to ensure comfort and dignity throughout the care journey.

Qualifications

• Valid RN license in good standing in the state you will work in.
• Valid driver’s license and reliable transportation.
• CPR certification (current).
• Strong observational and communication skills—written and verbal.
• Flexible, team-oriented attitude; willingness to support additional duties.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Provide one-on-one support to patients and families ensuring comfort and dignity.
• Conduct assessments of physical, emotional, and spiritual needs.
• Monitor and document vital signs and changes in condition.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
