Bristol Hospice is seeking an RN Case Manager to provide compassionate, whole-person care to patients and families in Orange County, CA. The role focuses on creating individualized care plans, advocating for patients, and guiding families through the end-of-life journey.

As a key member of a mission-driven team, you will collaborate with interdisciplinary staff, document progress, and ensure comfort and dignity in daily care.

Qualifications

• Valid RN license in good standing in the state you will work in.
• Valid driver's license and reliable transportation.
• CPR certification (current).
• Strong observational and communication skills—written and verbal.
• Flexible, team-oriented attitude and willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on patient history and current needs.
• Provide attentive, one-on-one support to patients and families, ensuring comfort and dignity.
• Conduct assessments of physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs and health changes; respond promptly as needed.
• Educate patients and families on care techniques and symptom management.
