The Pennant Group, Inc. through Guardian Hospice in Sherman, TX is seeking a licensed registered nurse to lead home hospice care in patients' homes and communities.

The RN will assess needs, develop care plans, and coordinate with families and the interdisciplinary team to deliver compassionate end-of-life care. The ideal candidate has at least two years of hospice or home health nursing experience, strong assessment and communication skills, and the ability to work independently with minimal

Competenze

• Graduate of NLN-accredited nursing school.
• Texas RN license required; must be current.
• Minimum two years’ hospice, home health, or public health nursing preferred.
• Ability to supervise hospice aides.

Mansioni

• Completes initial and ongoing assessments of patient and family to determine hospice needs.
• Provides comprehensive physical assessment and health history.
• Uses nursing process to deliver professional care.
• Develops and implements a care plan based on nursing diagnosis.
• Coordinates patient care as Case Manager for assigned caseload.
• Records clinical notes and communicates with physicians and the Interdisciplinary Group.
• Provides health education to patient and family; teaches self-care techniques.
