Job summary

The Pennant Group, Inc. is seeking a Registered Nurse to plan, organize and direct hospice care in patients' homes and communities, emphasizing education and community health. The role centers on coordinating care with the interdisciplinary team to meet patient and family needs.

You will perform patient assessments, develop individualized care plans, and provide education. On-call duties and driving to home visits are required, with strong emphasis on safety and quality of care.

Qualifications

• RN license in Oklahoma is required.
• Minimum two years in public health, home care, or hospice nursing.
• Current CPR certification.
• Valid driver's license and access to reliable transportation.
• Ability to lift and move patients and work independently.
• Ability to coordinate care with an interdisciplinary team.

Responsibilities

• Completes initial, comprehensive and ongoing patient/family assessments.
• Develops and implements care plans with goals based on nursing diagnoses.
• Administers medications and treatments as prescribed.
• Provides patient and family education on care and self-care techniques.
• Performs ongoing patient status evaluations and revises plans as needed.
• Acts as case manager and coordinates care across the caseload.
• Participates in on-call duties and ensures equipment and services are arranged.
• Maintains accurate clinical notes and communicates with physicians.
