CHRISTUS Hospice and Palliative Care SPOHN is seeking a Registered Nurse Case Manager to plan and deliver hospice care for patients in home and community settings. You will develop and implement the Plan of Care, collaborating with the care team to address physical, psychosocial, and spiritual needs.

Qualifikationen

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current CPR certification.
• Valid driver's license with access to reliable transportation.
• Ability to function in diverse home environments.

Aufgaben

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with the patient, family, and care team.
• Documents problems, goals, interventions, and responses to hospice care.
• Collaborates with patient/family, physician, and IDG members for daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes and quality documentation.
