Job summary

Asa Registered Nurse Hospice Case Manager with Spero Hospice in New Jersey, you will coordinate comprehensive hospice services and advocate for patient and family needs.

You will collaborate with an interprofessional team to develop care plans, educate families, and ensure regulatory compliance while delivering compassionate, holistic care that enhances quality of life.

Qualifications

• Active RN license in relevant state.
• Hospice or palliative care experience preferred.
• Strong communication, assessment, and care coordination skills.
• Ability to work collaboratively within an interprofessional team.
• Commitment to patient-centered, holistic care.

Responsibilities

• Coordinate and manage hospice care plans for assigned patients.
• Assess patient needs and develop individualized care plans with interdisciplinary team.
• Provide compassionate direct patient and family support, education, and advocacy.
• Collaborate with physicians, social workers, and spiritual care providers.
• Ensure compliance with regulations, documentation, and quality standards.
