Christus Hospice Schumpert, part of LHC Group and within the Optum family, seeks an RN Case Manager to oversee hospice patient care at home. You will develop and implement plans of care, collaborate with the care team, and support families through the health journey with compassionate, evidence-based nursing.

You will join a system committed to quality care, clinical excellence, and a culture of Caring. This role emphasizes patient-centered planning and timely evaluations in a home setting.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• LA state: 2+ years of full-time RN experience; hospice experience can substitute for RN experience.
• Preferred: 1+ years of clinical experience; CPR certification within 90 days of hire.

Responsabilités

• Make initial nursing evaluation within 48 hours of referral.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, and patient response.
• Collaborate with patient/family, physician, and IDG for daily care.
• Instruct patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes; function in diverse home environments.
