Job summary

Heart to Heart Hospice in Indiana is seeking a Patient Care Manager - Registered Nurse to lead clinical departments and ensure continuity, quality and safety of services in line with state and federal regulations. This role requires coordinating care and guiding a multidisciplinary team to deliver compassionate hospice care in the community.

Qualifications

• Current State Registered Nurse License.
• Two years of hospice RN experience in clinical care or home health.
• Must be highly motivated, self-directed, flexible and able to work in a multidisciplinary group.
• Strong organizational, people, problem solving, and multitasking skills.
• Strong critical thinking and communication abilities.

Responsibilities

• Direct and coordinate clinical departments and sites; ensure continuity, quality, and safety in compliance with regulations.
• Supervise nursing staff and guide team to meet the Plan of Care and reassessment needs.
• Support employee and patient education to improve knowledge and skills; train staff for high-quality care.
• Provide coaching, development, feedback and annual evaluations to team members.
• Ensure timely submission of all documentation.
• Ensure overall compliance with laws and Medicare regulations; follow personnel policies.
