Job summary

Heart to Heart Hospice in Midland, TX seeks an experienced RN Case Manager to conduct initial and ongoing assessments of patients with terminal illness and assess physical, psychosocial, and environment factors affecting ADLs.

You will implement the individualized plan of care, coordinate with physicians and IDG, participate in care planning, and provide on-call support to patients and families while maintaining accurate documentation.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of RN experience (preferred in healthcare/clinical setting).
• Current Driver's License and reliable transportation.

Responsibilities

• Initial and ongoing comprehensive assessments of the patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and revise the plan as necessary.
• Collaborates care amongst patient, caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate care planning with the IDG.
• On-call responsibilities and support to patients/families as assigned.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws and Medicare regulations.
