Job summary

The Care Team Hospice is seeking a dedicated RN Case Manager to provide skilled nursing care within a hospice framework in Midland, Michigan. You will assess patients, develop individualized care plans, and coordinate with physicians and families to ensure comfort and quality end-of-life care.

Responsibilities include documentation in HCHB, on-call rotation as needed, and collaboration with an interdisciplinary team to support patient-centered outcomes.

Qualifications

• Active RN license required.
• Hospice or home health experience preferred.
• Strong clinical assessment and care coordination skills.
• Ability to work independently in the field.
• Strong communication and documentation skills.
• Experience using HCHB (Homecare Homebase) preferred.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing care in accordance with hospice standards and physician orders.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Educate patients and families on symptom management, disease progression, and end-of-life care.
• Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB.
• Participate in interdisciplinary team meetings and care planning; support after-hours needs during on-call rotations when applicable.
