Job summary

AdventHealth is seeking an experienced Registered Nurse to provide hospice and home health nursing services in a patient-centered setting. You will coordinate care, administer medications, and support families across the care continuum.

The role requires strong clinical assessment, documentation in EMR, and collaboration with physicians and the care team to ensure high-quality outcomes in the home environment.

Qualifications

• Experience in home health nursing or hospice care preferred.
• Ability to assess and monitor patient conditions in the home setting.
• Familiarity with InterQual criteria and care planning.
• Strong communication with families and care team.

Responsibilities

• Completes documentation in the electronic medical records system.
• Provides comfort and support to family members and caregivers.
• Participates in on-call rotations for after-hours clinical needs.
• Manages Plan of Care for assigned caseload of patients.
• Educates patients and families on disease processes and safety.
• Collaborates with physicians and multidisciplinary team to update care plans.
