Job summary

Moments Hospice seeks an experienced RN Case Manager to provide compassionate, patient-centered care on the road. You will assess patients, adapt care plans, and coordinate with families and the healthcare team to ensure comfort and quality of life in final days and weeks.

Qualifications

• Licensed as a Registered Nurse in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
