Moments Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered care across the care continuum. Qualifications

• Licensed as a Registered Nurse in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
