Job summary

Moments Hospice seeks an experienced RN Case Manager to provide compassionate patient care and coordinate clinical plans in collaboration with families and the healthcare team.

You will conduct assessments, refine care plans, and ensure seamless care delivery across the hospice spectrum. A strong nursing background and EMR proficiency are required, with a valid driver’s license and insured vehicle for travel.

Qualifications

• RN license in the state you work in.
• Experience with Electronic Medical Records (EMR) software.
• Strong planning, organizing and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open communication with patients, families, and the healthcare team.
• Coordinate patient care for your caseload.
• Participate in interdisciplinary team meetings for care planning.
