Job summary

Moments Hospice in Indiana seeks an experienced RN Case Manager to deliver compassionate, high-quality in-home and on-road patient care. You will coordinate care plans, educate families, and work closely with our clinical team to ensure comfort in patients' final days.

Qualifications

Must be licensed as a Registered Nurse in the state of practice.
Experience with EMR systems.
Strong planning, organizing, and communication skills.
Ability to work autonomously while maintaining high professional standards.

Responsibilities

Conduct initial client assessments and refine care plans.
Provide hands-on nursing care and patient education.
Maintain open lines of communication with patients, families, and the healthcare team.
Manage patient care coordination for your caseload.
Participate in interdisciplinary team meetings for comprehensive care planning.
