Job summary

LifeSource Hospice in Bergen County, NJ is seeking an experienced RN Hospice Case Manager to lead a patient-centered hospice program. You will manage a caseload, conduct assessments, and coordinate care across disciplines.

In this field-based role, you will travel to patient homes, document thoroughly in WellSky EMR, and ensure CHAP-compliant standards while supporting patients and families through end-of-life care.

Qualifications

• Active NJ RN license in good standing.
• Minimum 1 year experience in hospice, home health, or long-term care.
• Familiarity with CHAP hospice standards preferred.
• Experience with WellSky EMR and BetterRx a plus.
• Strong clinical assessment and communication skills.
• Compassionate, patient-centered approach.
• Reliable transportation and valid driver's license.
• Ability to work independently and manage time effectively.

Responsibilities

• Manage a hospice caseload of approximately 15 patients.
• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing visits and symptom management.
• Coordinate care with physicians, social workers, chaplains, aides, and other IDT members.
• Ensure documentation meets CHAP standards and regulatory requirements.
• Utilize WellSky EMR for timely and accurate charting.
• Perform medication reconciliation and coordinate e-prescribing via BetterRx.
• Provide patient and family education related to disease progression and end-of-life care.
• Participate in IDT meetings and care planning.
• Collaborate with facilities and community partners.
• Travel to patient homes and facilities within Bergen County.

Position Summary

LifeSource Hospice is seeking a compassionate and experienced RN Hospice Case Manager to join our interdisciplinary team serving patients in Bergen County, NJ. This role is responsible for managing a caseload of hospice patients, coordinating care across disciplines, and ensuring compliance with hospice regulations and standards.

The ideal candidate has hospice, home health, or long-term care experience and is comfortable working independently in the field.

Schedule

• Full-time

• Monday-Friday

• On-call rotation (one weekend per month in Bergen county vicinity)
