Zusammenfassung

Interim HealthCare is seeking a Hospice RN Case Manager to provide high-quality, in-home nursing care for terminally ill patients in Michigan. You will work autonomously within a supportive team to coordinate care with physicians and CNAs.

Qualifikationen

• RN license active in Michigan.
• At least 1 year of RN clinical experience (Hospice preferred).
• Valid CPR certification.
• Reliable transportation with a valid driver's license and car insurance.

Aufgaben

• Provide home-based care to patients with terminal illness.
• Work with a team of physicians and oversee CNAs providing patient care.
• Examine patients, perform assessments, document and report changes to the physician.
• Educate patients on plan of care, medications, diet and exercise.
• Coach family members on plan of care, medications and home safety.
• Ensure goals are met and coordinate discharge from services.
