Job summary

Interim HealthCare in Michigan is seeking a Hospice RN Case Manager to deliver home-based nursing care to patients in their homes. The role offers autonomy, collaboration with physicians, and a focus on meaningful, one-on-one patient relationships.

Qualifications

• Active Michigan RN license required.
• Minimum 1 year of clinical RN experience; hospice experience preferred.
• CPR certification required.

Responsibilities

• Provide home-based hospice nursing care to patients.
• Oversee CNAs and coordinate patient care with physicians.
• Conduct assessments, document changes, and report to physicians.
• Educate patients and families on plans of care, medications, diet and safety.
• Coordinate discharge planning and ensure goals are met.
