Interim HealthCare in Michigan is seeking an experienced Registered Nurse to join as Hospice RN Case Manager. You will provide skilled, compassionate home-based care to patients with terminal illness and coordinate with physicians, CNAs, and families to implement the plan of care.

Qualifications

• Active MI RN license (unrestricted)
• Minimum 1 year clinical RN experience (Hospice preferred)
• Current CPR certification
• Reliable transportation and valid MI driver's license

Responsibilities

• Provide home-based care to patients with terminal illness
• Oversee CNAs and coordinate patient care with physicians
• Perform patient assessments and document changes
• Educate patients and families on plan of care, medications, and home safety
• Coordinate discharge from services
