Trinity Hospice 1 LLC is seeking an experienced RN Case Manager to deliver comprehensive hospice nursing care in Chesapeake, VA. You will assess patients and families, develop individualized care plans, and coordinate with an interdisciplinary team to ensure comfort and quality of life.

The role requires an active RN license in the state and prior hospice case management experience. A full benefits package and paid holidays are included, with on-site responsibilities in Chesapeake, VA.

Responsabilités

• Perform comprehensive nursing assessments to identify physical, psychosocial, and emotional needs.
• Develop, implement, and evaluate patient-centered care plans focusing on comfort and quality of life.
• Provide skilled nursing interventions: pain management, wound care, medication administration, clinical monitoring.
• Coordinate care as the primary RN case manager across the interdisciplinary team.
• Collaborate with physicians, social workers, chaplains, and hospice aides for seamless care delivery.
• Facilitate timely communication to support care planning; maintain thorough documentation per standards.
• Participate in IDT meetings, care conferences, and ongoing care planning.
