Job summary

Hospice of New Mexico is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our nursing team in Albuquerque. This full-time role delivers direct hospice care, coordinates services, and guides patients and families through the hospice journey with a focus on dignity, comfort, and compassionate connection.

Responsibilities include engaging with clinical referrals, collaborating with the Clinical Director, educating families, and participating in call duties to ensure timely,

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries with compassion.
• Collaborative Care Champion: Partner with Clinical Director to assess eligibility and suitability for hospice services.
• Nurturing Direct Care: As an RN Case Manager, coordinate care while ensuring patient comfort and quality of life.
• Nursing with Heart: Deliver nursing care rooted in proven principles to ease pain and provide solace.
• Educator and Advocate: Educate patients and families to empower participation in care plans.
• Call of Compassion: Participate in after-hours call duties as part of the team.
