The role requires Texas RN license, valid driver’s license, and hospice/case management experience; on-call rotation and collaboration with the interdisciplinary team. A $5,000 sign-on bonus is offered along with a comprehensive benefits package.

Qualifications

• Requires Texas RN license and valid driver's license.
• Hospice/case management experience preferred.
• Proficient with computer and MS Office applications.

Responsibilities

• Provide professional nursing care using the nursing process.
• Coordinate admission and plan of care for patients.
• Develop and communicate care plans with patients and families.
• Administer medications and treatments as prescribed.
• Collaborate with the interdisciplinary team and participate in on-call rotation.
