Community Healthcare of Texas in Dallas, TX is seeking a Registered Nurse (RN) On Call to provide emergency support and skilled visits during non-standard hours. This full-time position requires Texas RN licensure, valid driver's license, and at least 2 years of RN experience with on-call exposure.

Qualifications

• Registered Nurse in the state of Texas.
• Hold a valid driver’s license and have a dependable means of transportation.
• Minimum 2 years of RN experience required.
• 6+ months in an On-Call role required.
• Crisis management experience strongly preferred.
• 1 year Hospice or Home Health experience strongly preferred.
• Proficient computer skills; Microsoft Office suite experience preferred.

Responsibilities

• Make emergency on-call visits providing nursing care per the care plan.
• Document to Medicare conditions of participation; provide symptom management and support.
• Participate in weekly team conferences with required information and documentation.
• Provide care for Hospice patients during non-office hours; ensure security via phone and visits.
