Job summary

Community Healthcare of Texas has provided hospice and palliative care since 1996. We are currently seeking an RN Case Manager to meet with patients and families, explain the scope of hospice services, coordinate admissions, and manage ongoing care plans.

The role requires Texas RN licensure, a valid driver’s license, and 1 year of nursing experience; hospice case management experience (2+ years preferred) and MS Office proficiency are valued. A $5,000 sign-on bonus is offered.

Qualifications

• Texas RN licensure required.
• Valid driver’s license and reliable transportation.
• 1 year of nursing experience.
• Hospice case management experience a plus (2+ years preferred).
• Computer skills, including MS Office, required.

Responsibilities

• Meet with patients and families to explain hospice scope and coordinate admissions and ongoing care plans.
• Assess patient status and develop, initiate, and revise the plan of care.
• Administer prescribed medications and treatments.
• Coordinate patient care as Case Manager with the clinical team.
• Document clinical notes and track pain/symptom changes.
