Job summary

Luminary Hospice is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care in patients’ homes and communities. The role emphasizes community health education and coordination with families to meet end-of-life needs.

Responsibilities include comprehensive assessments, developing and revising care plans, and collaborating with an interdisciplinary team to ensure compassionate, high-quality care. On-call duties and driving to patient locations are involved.

Qualifications

• Graduate of an accredited nursing program.
• Current licensure in state and CPR certification.
• Bachelor’s degree with hospice/home care experience preferred.
• Management experience not required; able to supervise hospice aides.

Responsibilities

• Performs initial and ongoing comprehensive patient/family assessments.
• Develops and initiates a plan of care with goals based on nursing diagnosis.
• Coordinates patient care with interdisciplinary team and community resources.
• Administers medications and treatments as prescribed; provides patient/family education.
