Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications.
• Knowledgeable about quality assessment procedures.

Responsibilities

• Manage patient and family caseload across assessment, eligibility, admission, planning, implementation and evaluation stages of the nursing process.
• Gather data from patient and family across physical, psychological, social and spiritual factors to inform care.
• Coordinate care by communicating with the attending physician and interdisciplinary team.
• Provide regular visits to reassess needs and update the care plan.
• Perform clinical pronouncement of death and support families.
• Maintain accurate patient records and prepare care plans in a timely manner.
• Supervise Home Health Aide delivery of care per guidelines.
