Advocate Health in Wisconsin is seeking an experienced Registered Nurse to coordinate end-of-life care for hospice patients and families. You will educate families, monitor patient progress, and collaborate with the interdisciplinary team to maintain optimal care and comfort.

The ideal candidate has 1–2 years in nursing, is proficient with electronic health records, and holds a valid RN license with BLS certification.

Qualificações

• RN license required and BLS for Healthcare Providers certification preferred
• Completion of an accredited nursing program.
• 1–2 years nursing experience in home health, hospice, or related setting recommended
• Strong assessment, communication, and documentation skills.

Responsabilidades

• Identify and coordinate patient/family care to support hospice patients and families; contact at least every 15 days.
• Educate patients and families on disease processes, hospice care, and care trajectories; update physicians as required.
• Review caseload to ensure appropriate level of care and make referrals as needed; administer medications as prescribed.
