Job summary
RCM Healthcare Services is seeking an experienced Hospice Field Case Manager / RN (Registered Nurse) in New York, NY area. The role includes nursing assessments, care plan development, and coordination with physicians for orders, with supervision of home health aides. Eligible candidates will have NY RN licensure, strong bedside nursing, and a willingness to train.
Excellent benefits and a $15,000 sign-on bonus are offered. Territories available: Bronx or Manhattan.
Qualifications
- Two years of Community or Oncology nursing experience.
- Proficiency with Word and Excel.
- Bilingual abilities are a plus.
Responsibilities
- Visit hospice and palliative care patients in community and facility settings to perform physical and psychosocial assessments, and document visits electronically.
- Develop, implement, and update individualized nursing care plans based on patient needs.
- Coordinate care changes with physicians by requesting written orders when needed.
- Supervise, train, and evaluate home health aides and homemakers supporting hospice patients.
- Follow the required schedule, including one weekend per month and backup on-call coverage 2–3 times monthly, with car and escort service provided for on-call shifts.
